Erection Reversal Protocols and Management of Prolonged Erections

Prolonged erections and priapism are considered the most severe complications of penile Doppler ultrasound (PDU) studies because it might lead to irreversible compromise of erectile tissue. It is essential to reassess erection rigidity in the end of each PDU examination and to ensure adequate penile detumescence, as no patient may be discharged with a rigid erection. Therefore, PDU examiners must be acquainted with pharmacological principles of penile detumescence and be trained in penile aspiration procedures. This chapter aims to review the most relevant aspects of sympathomimetics use during PDU to allow for safe erection reversal or early penile aspiration whenever necessary.
