Renal and Ureteric Trauma
摘要
The kidney is the most commonly injured genitourinary organ. Renal trauma can be acutely life-threatening, but the majority of renal injuries can be managed non operatively. Advances in the imaging and in treatment strategies during the last 20 years, have decreased the need for surgical intervention and increased renal preservation. Hemodynamically stable patients with visible hematuria should undergo a CT scan with intravenous contrast. Patients not responding to resuscitation are candidates for exploration. Ureteral trauma is rare due to anatomical reasons. Ureteral injuries may be partial or complete and may be caused by blunt or penetrating trauma. Iatrogenic injuries are the most common form of trauma to the ureter. A delay in diagnosis is the most important contributory factor in morbidity related to this kind of injury. Contrast enhanced CT scan approaches 90% diagnostic accuracy. Reconstruction is feasible in most cases.
