Fournier’s disease (FG) is a rare progressive fulminant necrotizing disease mainly involving the genital, perineal, and perianal areas. High BMI and diabetes are the most frequent risk factors. Clinical diagnosis is based on symptoms, such as pain, prostration, fever, scrotal skin swelling, and foul-smelling exudate production. The LRINEC score may be helpful for diagnosis whilst the FGSI and UFGSI scores for prognosis and to ascertain the risk of mortality. Immediate empirical multimodal antibiotic treatment and a large debridement of necrotic tissues should be started at diagnosis to reduce the risk of disease progression, sepsis, and death. Microbiological investigations on tissues and exudates should be performed to confirm bacterial strain sensitivity to antibiotics. The effects of vacuum-assisted closure (VAC) and hyperbaric oxygen therapy (HBT) as supportive treatments for reconstruction must still be confirmed by appropriate randomized controlled trials.
