Stroke is a leading cause of morbidity and mortality, and imaging has evolved as a critical tool for diagnosis and treatment guidance. Noncontrast CT assesses the extent of ischemic changes, for the presence of intracranial hemorrhage and for stroke mimics. MRI demonstrates a higher sensitivity and specificity for early or small acute infarcts and petechial parenchymal hemorrhage and can help determine the timing of stroke onset. Advanced imaging such as CT perfusion or MR perfusion assess the size of the ischemic core and penumbra and assists in patient selection for endovascular thrombectomy.
