Considerations for Emerging and Young Adults with Chronic Childhood Conditions

Emerging adults with chronic childhood-onset conditions are vulnerable to poor health outcomes, and it is imperative that they have optimal resources and support to help them succeed during their transition to adult care. Several models of care have been used for these patients, including patient-centered medical homes and the chronic care model. The emerging adults’ primary care physicians play an essential role in their care and should follow 10 important steps to the best care for their patients, including recommendations from six core elements of Got Transition. Special considerations should be given to patient’s social determinants of health, cultural, and religious considerations, developmental conditions, and local transition resources.
