Institutional Considerations for Transitions of Care

Transition of care: The process in which adolescents change from receiving medical care by pediatricians and pediatric specialists to receiving care from primary care providers and specialists focused on the care of adults. Inpatient transition guidelines can provide a framework and process for thoughtful and effective transition of care between pediatric and adult hospitalists and specialists. This chapter provides a framework for creating an inpatient transition process guideline.
