Comprehensive geriatric assessment (CGA) is an interdisciplinary team-based approach to address the multiple domains of health in older people. CGA utilizes an interdisciplinary team approach to assess the older adult, tailored to the setting. Domains addressed include physical and mental health, comorbidities, screening for disorders, assessment of functional status, and measures to prevent illness. Relevant areas addressed include patient safety, quality of life, socioeconomic and environmental factors. Timely discussion of advance care planning and implementing directives are part of CGA. Evaluation of cognition and mood, and determination of capacity are fundamental. Periodic medication review, reconciliation, and deprescribing are included. Utilizing CGA is beneficial and improves outcomes for health, besides delaying or avoiding hospitalization and institutionalization. CGA and recommendations are tailored to health status, life expectancy, patient preferences, and setting of care. An Annual Wellness visit offers opportunities for CGA and implementation of several measures. The concept of Age-Friendly Health and 4Ms including Medications, Mobility, Mentation, and what Matters is now incorporated as part of the assessment. The American Geriatrics Society Choosing Wisely principles are ideal components of good practice.
