Autologous Conditioned Serum (ACS)

In the last decades, local injections of autologous blood preparate begin to be a solid option in the conservative treatment of the musculoskeletal injuries and degenerative changes. Autologous conditioned serum (ACS) therapy is a promising regenerative treatment modality in selected patients. ACS’s ability is to modulate inflammation, promote tissue repair, and stimulate the regenerative capacity of the affected site. The therapeutic effects of ACS therapy are mediated through various mechanisms, including the modulation of inflammation, growth factor, and cytokine regulation. The main cytokine in the ACS is IL-1Ra which inhibits the binding of interleukin-1 (IL-1) to its receptors. By blocking the IL-1 signaling pathway, ACS therapy reduces inflammation and mitigates its detrimental effects on tissues. In addition to IL-1Ra, ACS also contains other anti-inflammatory cytokines, such as IL-4, IL-10, and IL-13, which further contribute to the downregulation of inflammatory responses. Autologous conditioned serum (ACS) therapy has demonstrated promising clinical applications in various musculoskeletal disorders. ACS therapy has shown promising results in osteoarthritis, tendinopathies, ligament injuries, postoperative soft tissue surgeries, and epidural injection for back pain. Indications, dosages, and volume of doses vary with the application area. There have been no serious side effects attributed to the ACS therapy. Attributed side effects are the same as other injections which are heat, swelling, and pain at the application area. Future research should focus on optimizing the treatment protocols for ACS therapy. This includes determining the optimal concentration of growth factors and cytokines in ACS, refining the processing techniques to maximize their bioavailability, and identifying the most effective dosing regimens.
