Dystonia

The chapter delineates the complexity and nuances of diagnosing and managing dystonia, a hyperkinetic movement disorder phenomenology characterized by abnormal posturing and often misinterpreted due to its multifaceted phenomenological features. Establishing a thorough history and conducting a comprehensive examination are crucial to ascertain the etiology and dynamics of dystonia, which can manifest acutely or over a prolonged period and may present with task-specificity, sensory tricks, state function behavior, mirror dystonia, or a null point. An understanding of progression and potential spread across body regions provides a useful clue for the diagnosis of various forms of dystonia such as DYT1 and DYT12 dystonias. Therapeutic approaches, including the application of pharmacotherapy, botulinum toxin, as well as deep brain stimulation, have transformed management, albeit presenting challenges in tailoring individualized treatment strategies.
