Bimaxillary Surgery

With the help of bimaxillary surgery, it is possible to reposition the upper and lower jaws so that they are in a functionally and aesthetically harmonious relationship. The most important variables to be adjusted are the hard and soft tissues so that the resulting outcome is a very good occlusion, a harmonious facial profile, and a good masticatory function. The soft tissue follows the hard tissue but to varying degrees. A sole cephalometric assignment of the jaw bases is therefore inadmissible, as this can lead to aesthetic losses in the postoperative soft tissue. In addition, bimaxillary realignment osteotomy requires special care and knowledge of the surgical area due to its adjacency to delicate anatomic structures. The most important surgical techniques for bimaxillary surgery are the Le Fort I osteotomy and the bilateral sagittal split osteotomy (BSSI). Surgical advancement has received recent attention with improved techniques, advances in anesthesia, improved osteosynthesis, and a better understanding of the impact on the hard and soft tissues. The newly established virtual planning also adds modern aspects to this procedure that has been performed for more than 150 years.
