Application of the Risk Matrix Method to Brachytherapy Treatments

Radiotherapy treatments involve highly complex processes where many errors or near misses may occur. Some may be caused by equipment failures, but many others are due to different kinds of human errors during the workflow. For this reason, risk analysis is crucial and the application of the risk matrix method seems to be a tool of undeniable necessity. In this project, we follow the international recommendations of the AAPM (TG100) and FORO to define and organize the risk associated with the initiating events identified at each stage of brachytherapy treatment. Based on the outcome, it was possible to make safety recommendations in order to avoid or prevent such events and finally reduce the risk.
