Perspectives of Immunotherapy in Prostate Cancer

Prostate cancer, a major twenty-first-century health concern, often remains asymptomatic in its early stages but can lead to urinary, pelvic, and erectile problems as it progresses. It ranks among the most prevalent global cancers, with over 248,000 new cases annually in the USA and stands as the second leading cause of cancer-related deaths in men, primarily in North America, Europe, and Australia. Several risk factors, such as age, family history, race, diet, obesity, and physical inactivity, contribute to prostate cancer’s development. Diagnosis relies on a combination of blood tests, imaging, and biopsy, with the prostate-specific antigen (PSA) test being a crucial screening tool. Treatment options range from surgery and radiation for early-stage cancer to hormonal therapies, chemotherapy, immunotherapy, targeted therapy, and theranostics for advanced cases. Immunotherapy, an innovative cancer treatment, has emerged as a significant player in prostate cancer management. It harnesses the immune system’s power to selectively target cancer cells. However, prostate cancer, especially metastatic castration-resistant prostate cancer (mCRPC), presents challenges for immunotherapy due to its immunologically “cold” nature and resistance to immune checkpoint therapy. Immunotherapy encompasses various approaches, including checkpoint inhibitors, cancer vaccines, adoptive T cell therapies, monoclonal antibodies, oncolytic virus therapy, and cytokines. Checkpoint inhibitors like pembrolizumab have shown promise in clinical trials, gaining approvals for mCRPC patients. Cellular immunotherapy, exemplified by sipuleucel-T, utilizes the patient’s immune cells to target cancer cells. Sipuleucel-T has been a significant breakthrough, demonstrating improved overall survival in mCRPC patients. Cellular vaccines like rilimogene galvacirepvec and GVAX are also under investigation, showing promise in early-phase trials. Despite these advancements, challenges persist. Immunotherapy’s effectiveness varies among patients, prompting research into personalized approaches and combination therapies. Additionally, while immunotherapy offers advantages like long-term control and fewer side effects, it can trigger autoimmune reactions. In conclusion, immunotherapy is revolutionizing prostate cancer treatment by leveraging the body’s immune system. Ongoing research aims to enhance its effectiveness, tailor treatments to individual patients, and explore combination therapies. Immunotherapy holds immense promise for improving outcomes in prostate cancer patients worldwide.
