The pharmaceutical industry has a profitable but precarious operating model. Until the product launch, the cash flow is negative due to the R&D expenses. Upon launch, the drug sales bring in revenues, and cash flow becomes positive. This business model has been around for a long time, but in recent times, the prices and profits in the biopharmaceutical industry have been increasingly scrutinized in social debate. With the passage of the Inflation Reduction Act of 2022 and a concerted effort worldwide to move towards value-based pricing systems, the marketplace for pharmaceuticals is evolving rapidly as most governments struggle to balance health and social expenditures at sustainable levels. Therefore, the product designers must be aware of the market dynamics that might most likely exist when their products enter the marketplace. The practical reality is that the pharmaceutical industry may need to account for a future where value-based pricing is the norm and not the exception. In such a scenario, designers of pharmaceutical products will routinely be asked what can be (further) done to reduce the costs so that the product does not cross the cost-effectiveness threshold. Situational awareness of the ever-changing and evolving healthcare marketplace is thus paramount for pharmaceutical scientists.
