This chapter focuses on the vital role of advance care planning (ACP) in healthcare. At the heart of ACP is the effort to understand each patient’s story, including their values and what matters most to them, as these values and goals have important implications for their preferences for medical care both in the moment and in the future. This requires deep listening, empathy, and a careful approach to complex feelings and spiritual matters. By truly understanding patients’ stories, healthcare professionals can work together with the patient and their loved ones to create plans for care that reflect their values, improving the quality of communication and helping patients and their loved ones feel confident in making decisions about care. We look into key communication strategies which healthcare professionals can apply for an effective ACP process. This includes dealing with cultural and spiritual issues sensitively, managing complex situations well, and working closely with other professionals. This chapter provides an overview of these strategies, offering healthcare professionals the guidance needed to implement ACP in a respectful and patient-focused way.
