Hypertrophic cardiomyopathy (HCM) is an inherited heart muscle disorder and a leading cause of sudden cardiac death (SCD) in young adults. It affects about 1 in 500 adults in the general population. This chapter describes the first risk model developed to predict the risk of SCD for HCM patients. The risk model has been incorporated into the 2014 European Society of Cardiology (ESC) guidelines and is used worldwide by cardiologists as a risk stratification tool during clinical assessment of HCM patients, to decide on the best treatment option for individual patients. The online risk calculator features on the ESC website. The HCM Risk-SCD model has improved predictive ability compared to the old clinical guidelines offering an optimal treatment decision making tool by balancing risks of SCD and ICD complications. It is a major landmark in the management of cardiomyopathy.
