A Salter-Harris 3 fracture through the lateral aspect of the distal tibial epiphysis is known as a Tillaux fracture (after the French surgeon and anatomist). This fracture is associated with an avulsion of the anterior tibiofibular ligament following an abduction-external rotation mechanism of injury. These injuries occur more often in older children and adolescents, at which age the medial aspect of the distal tibial physis has usually fused. Remember that the distal tibial physis fuses from medial to lateral, i.e. the medial aspect of the distal tibial physis fuses before the lateral aspect, with the latter fusing at around 12–15 years of age.
