Children under the age of 1 year, and in particular those less than 4 months, have the highest incidence of metaphyseal and abusive fractures. Classical metaphyseal lesions (CML) are less common in older children who are able to walk. The commonest locations for metaphyseal fractures include the distal femur, the proximal and distal tibia, and the proximal humerii. These fractures are reported to be the commonest long-bone fracture in fatally abused infants, specifically bilateral asymmetric tibial CMLs.
