The open partial supratracheal laryngectomy (OPHL III) according to the ELS classification is a subtotal laryngectomy. It can be considered as an alternative to total laryngectomy for the treatment of selected locally intermediate-to-advanced laryngeal cancers (cT2–3–4a) with subglottic extension, and/or for tumors with posterior extension through the thyrocricoarytenoid space, if at least one cricoarytenoid unit can be spared. The oncological outcomes confirm the OPHL III effectiveness. The decannulation rate ranges from 60% to 90%. The present section describes step-by-step this surgical procedure.
