The OPHL type II, previously named supracricoid laryngectomy, is an open laryngeal organ preservation surgery indicated for T1a–T1b glottic tumors in patients with inadequate laryngeal exposure, T2 glottic tumors with impaired vocal cord mobility, limited extension to supraglottis and/or subglottis (sempre sopra il limite superior della cartilagine cricoide), selected T3 and T4a tumors anterior to a frontal plane passing through the vocal process of the arytenoid, perpendicular to the thyroid lamina (magic line), and selected recurrences after Transoral Laryngeal Microsurgery (TLM) or after radiation therapy. The surgical steps are extensively described in this chapter and culminate with the creation of a cricohyoidoepiglottopexy (OPHL IIA) or a cricohyoidopexy(OPHL IIB). A focus on the several complications that could be encountered and their management has been performed.
