Rescue Stenting After Failed Thrombectomy of a Large Vessel Occlusion Related to Calcified Cerebral Embolus

Reperfusion of large vessel occlusion caused by calcified cerebral embolism remains a challenge, and failure to achieve reperfusion results in poor clinical outcome. In this chapter, we describe the steps to perform permanent intracranial stenting to treat a refractory occlusion caused by a calcified cerebral embolus, illustrated by a case of major stroke caused by the internal carotid artery. Four passes of mechanical thrombectomy failed to achieve reperfusion. Rescue stenting with a Neuroform Atlas stent was attempted and successfully performed, resulting in complete reperfusion and a favorable clinical outcome.
