Combined Occlusion of the Middle Cerebral Artery with a Floating Thrombus at the Branching of a Persistent Primitive Trigeminal Artery of the Internal Carotid Artery

Anatomical variations are a common finding in the field of neurointervention but can also be challenging in certain situations. In this case, a patient presented with a middle cerebral artery occlusion combined with a floating thrombus at the bifurcation of a persistent primitive trigeminal artery. This chapter discusses the management of this unstable situation with a high risk of secondary embolization in two vascular territories and the actions taken to ensure reperfusion and a good clinical outcome.
