Chimneys and Periscopes

Chimney EVAR (Ch-EVAR) consists of the placement of covered stents parallel and outside of the main abdominal endograft maintaining the perfusion into the involved target vessels, improving the seal zone, and overcoming relevant anatomical obstacles by using standard low-profile and flexible off-the-shelf devices. We present the treatment of a rupture-threatening aortic pathology with challenging neck and co-existing hostile iliac access by use of intravascular lithotripsy (IVL) and double Ch-EVAR.
