摘要

It is vital that home healthcare providers develop and maintain a clear understanding of the patient and family caregiver’s needs, concerns, and perspectives. This understanding is necessary if positive health outcomes are to be accomplished for the older patient. Strategies for building and maintaining strong collaborative relationships with patients and families are explored in this chapter. Although most care models are patient-centered, providers must not disregard the pivotal role of the family caregiver. The caregiver is a valuable asset for home healthcare providers. This chapter concludes with the introduction to a case study that will be referenced throughout the book.
