Effective interprofessional collaboration is needed to support older adults in receiving and responding appropriately to home healthcare services. Moreover, the research literature underscores the numerous benefits of collaborating interprofessionally on behalf of older patients and their families. Theoretical frameworks and care models are needed to guide home care practices and service delivery. Quality indicators relating to the care of older patients in the home setting should be continuously identified and sustained to assure the best possible patient outcomes. Paramount to interprofessional collaboration is members’ commitment to respecting all team members, exhibiting cultural and professional humility, and working both cooperatively and diligently toward the attainment of identified treatment goals. This final chapter is a closing the loop of sorts. The authors integrate and synthesize central points from the book chapters in order to establish a strong and reliable platform for effective interprofessional collaborations.
