Diabetes is one of the most common chronic diseases worldwide. It constitutes a major public health concern that necessitates lifelong lifestyle modifications. Inadequate glycemic control may lead to acute complications and progressive damage to multiple organ systems, ultimately reducing individuals’ quality of life. To mitigate these risks, effective self-management of diabetes is essential. Transitioning from the clinic to home, it is recommended that individuals monitor their glucose levels, minimize acute complications such as hypoglycemia/hyperglycemia, continue their diabetes nutrition therapy, use oral antidiabetic drugs or insulin correctly, and follow up with their healthcare provider. Throughout this process, the involvement of family members or caregivers plays a vital role in supporting treatment adherence and improving overall diabetes management outcomes.
