Redefining Surgical Risk: Integrating Frailty Assessment into Resilience Management

Over the past 50 years, advancements in healthcare have significantly extended human life expectancy, but with these gains come challenges in managing aging-related chronic conditions, polypharmacy, and disabilities. Ensuring resilience—defined as the ability to adapt to stressors and recover—is now critical for optimizing surgical outcomes in older adults. The increasing need for surgeries among the older person has prompted a shift from relying solely on chronological age to assessing factors like frailty, physiological changes, and resilience. Frailty, marked by a decline in multiple physiological systems, combined with resilience, better predicts surgical outcomes and recovery potential than age alone. The “Surgery for Frails” approach prioritizes thorough preoperative assessments, interdisciplinary care, and collaborative decision-making. Evaluating both frailty and resilience—through tools like handgrip strength tests and assessments of mental health and social support—helps tailor interventions to patient needs. By integrating frailty and resilience assessments, healthcare teams can enhance prehabilitation, perioperative management, and recovery strategies. This model not only reduces mortality and improves quality of life but also optimizes healthcare costs, promoting better outcomes for the aging population.
