Codesign and Cocreation with People with Disabilities

People with disabilities (PwD) often face significant challenges in accessing appropriate healthcare and making use of health promotion services. Potential solutions are codesign, cocreation, and participatory approaches. Codesign involves collaborative design processes with stakeholders; cocreation extends this collaboration to the implementation and evaluation phase; and participatory health projects and participatory research generally engage PwD, patients, and the public in the research process, called cocreation. Theories and models are important to understand how effective and safe (or at least harmless) cocreation works. By these means, the effectiveness of collaborative efforts can be explained and ensured through mechanisms such as clear communication, shared decision-making and mutual goals, equal respect, and ongoing evaluation. Psychological processes and behavior change techniques, such as group dynamics, help to understand and improve this process. Employing and enhancing such techniques involves understanding and leveraging social influences, motivational factors, and supportive environments to facilitate behavior change. Ample experiences and scientific evidence demonstrate that cocreation can be effective in producing innovative solutions, which are well-aligned with the needs of end users, leading to improved health outcomes. However, researching this in a systematic and reliable way remains rather rare and difficult. For cocreation to be beneficial, it requires structured processes, active engagement, and the inclusion of diverse perspectives. At the same time, being aware of potential negative effects and collateral damage is important. To prevent negative effects and collateral damage, it is essential to manage expectations, address power imbalances, and ensure that all participants feel heard and valued. Successful cocreation can be facilitated by tools such as the ones developed by the HealthCascade network: social identity mapping, stakeholder analysis, collaborative planning frameworks, and continuous feedback. These tools help structure the process, foster engagement, and ensure the alignment of goals and actions.
