An abscess in the deep postanal space will extend into one or both ischiorectal spaces. Hanley devised a procedure based on the premise that when there is adequate drainage of the postanal space, lateral abscess fistulous extensions in the ischiorectal spaces from the postanal space will resolve spontaneously.
