Perianal Paget’s disease is an uncommon condition. A high index of suspicion with biopsy for histologic confirmation is required in order not to miss the diagnosis. Perianal skin mapping ensures negative margins of resection. Skin grafting is an acceptable method of perianal reconstruction.
