In 1880, Hodges coined the term pilonidal, “pilus” meaning hair and “nidus” meaning nest. Pilonidal disease occurs when hairs, trapped beneath the sacrococcygeal skin, create an abscess that may rupture, leaving a chronically draining sinus. The condition is seen most commonly in young adults. Anatomic features that predispose to the development of pilonidal disease include a deep natal cleft, a hirsute body habitus, obesity, moisture, bacteria, and midline depressions or “pits” that trap hairs. Pilonidal sinuses may be treated by unroofing.
