A case of anal high-grade squamous intraepithelial lesion in a patient with chronic colorectal inflammatory bowel disease is presented. Screening for anal dysplasia in patients with inflammatory bowel disease currently is not standard of care. Nevertheless, there is literature that shows an increased risk for anal squamous intraepithelial lesions among inflammatory bowel disease patients because of immunosuppression.
