Patella Tendon Repair Reconstruction for Failed Fixation

Patellar tendon (PT) ruptures are relatively uncommon injuries that can cause meaningful disability. When surgical repair is performed acutely, most patients will have good to excellent outcomes and return to activity levels comparable to before injury. Several surgical techniques are commonly used, with graft augmentation, particularly in the case of chronic injuries. In treating a chronic rupture or failed primary patella tendon repair, there is often significant scarring and tendon contraction/retraction complicating the repair. This frequently requires graft/biologic implants and lengthening procedures. Case: Our patient concerns a 32-year-old male with a history of right patellar tendon rupture status-post primary repair, presenting 8 years after re-rupture of his primary repair, with a chronic injury, significant retraction of the patella tendon, and a resulting disability.
