Alveoloplasty, a crucial aspect of tooth extraction procedures, aims to recontour irregular alveolar bone to create an optimal foundation for prosthetic work while preserving bone and soft tissues. This process, performed either concurrently with extraction (primary alveoloplasty) or after initial healing (secondary alveoloplasty), involves careful removal of bone irregularities. Various techniques, including simple alveoloplasty for single-tooth extraction and conservative alveoloplasty for multiple extractions, are employed. Instruments like rongeurs, bone files, and piezosurgery contribute to effective recontouring, with piezosurgery emerging as a preferred choice for its minimally invasive nature. The importance of meticulous flap reflection, proper irrigation, and suturing techniques is emphasized. Additionally, the intraseptal alveoloplasty technique, introduced by Dean, offers an alternative method for addressing irregularities without significant ridge reduction, enhancing long-term stability. Careful consideration of ridge form reconstruction is advised before bone removal. While this technique presents advantages in reducing labial prominence and preserving periosteal attachment, potential limitations regarding future implant placement must be considered. Overall, alveoloplasty plays a pivotal role in optimizing dental prosthetics and, when performed judiciously, contributes to long-term oral health.
