Preplastic Periodontal Surgery and Preimplant Plastic Surgery

The maintenance of periimplant health hinges significantly on the soft tissue phenotype surrounding dental implants, a subject of ongoing debate and research within the dental community. Avila-Ortiz et al.’s delineation of three crucial soft tissue components—periimplant keratinized mucosa width (KMW), periimplant mucosal thickness (MT), and periimplant supracrestal tissue height (STH)—has provided valuable insights into their pivotal roles. Inadequate KMW has been associated with increased plaque accumulation and inflammation, emphasizing its importance in periimplant health. Soft tissue grafting procedures, including free gingival grafts (FGG) and connective tissue grafts (CTG), have shown promise in augmenting soft tissue parameters, albeit with varying degrees of patient morbidity. Recent evidence underscores the potential of apically positioned flap surgery combined with soft tissue grafting in reducing periimplant complications. However, the impact of soft tissue phenotype modification on periimplant bone-level preservation remains uncertain, with inconsistent findings reported in the literature. The proposed ideal soft tissue phenotype parameters categorize KMW, MT, and STH into clinically relevant classifications, providing guidance for effective assessment and management. Soft tissue augmentation interventions, integrated into various phases of implant therapy, aim to mitigate periimplant biologic complications and enhance long-term tissue stability and esthetics. Harvesting approaches for soft tissue grafts have evolved to minimize donor site morbidity and optimize graft quality. While autogenous grafts remain the gold standard, alternatives such as soft tissue substitutes offer reduced invasiveness but lower efficacy. Clinicians face the challenge of balancing clinical efficacy with patient preferences and comfort when selecting the appropriate treatment approach. Complications associated with soft tissue augmentation procedures necessitate careful intraoperative management and postoperative care. Strategies to mitigate risks include meticulous surgical planning, monitoring for infection, and promoting hemostasis and wound healing. Despite advancements in soft tissue augmentation techniques, further research is needed to refine interventions and improve patient outcomes while minimizing complications.
