Redefining Gummy Smile Etiology and Treatment: An Insight into the Role of Chin and Peri-Oral Muscles

This chapter challenges conventional understanding of gummy smiles, typically characterized by excessive gingival display and traditionally treated via maxillary impaction or other types of surgical and non-surgical treatments. It introduces a novel categorization for gummy smiles, suggesting that their cause can be traced to both superior and inferior factors, including retrognathia. We tried to discuss regarding the issue from different aspects which have been discussed less or maybe never discussed earlier, the fact that gummy smile origin can be traced not only to superior factors like hyperactive muscles, vertical maxillary excess, or a short upper lip but also to inferior aspects such as retrognathia. The text proposes a shift in treatment strategies that considers the interconnected nature of peri-oral muscles and the influence of anatomical features, such as the mentalis muscle. An innovative application of Frank-Starling’s Law and a newly described “spring effect” provide insights into the musculoskeletal dynamics of the facial region. Also underscores the limitations and potential negative impacts of maxillary impaction, advocating for a more personalized and conservative approach that addresses the root cause of each gummy smile case.
