Reconstruction of the maxillary defect presents unique problems. A thorough understanding of the anatomy, structures, and functional deficits that need to be addressed through the reconstruction will allow the surgeon to select the proper free flap. This chapter highlights anatomic considerations, the differences in various classification systems, and a defect-based approach to reconstruction. In addition, factors such as pedicle considerations, pediatric concerns, and postoperative complications will be addressed.
