Management of Maxillary Hypoplasia in Cleft Patients

This chapter should aid in understanding the complex anatomy of cleft maxillary hypoplasia (CMH) and its management. This condition results from impaired maxillary growth, often due to intrinsic developmental deficiencies or scaring from primary surgical interventions. The chapter highlights the multidisciplinary approach required for managing CMH, encompassing orthodontics, growth modulation, and surgical interventions. It provides a comprehensive analysis of the etiology of maxillary growth deficiencies. The condition is categorized based on three main dimensions of deficiency—sagittal, transverse, and vertical—with associated clinical presentations like concave facial profiles, class III skeletal relations, and occlusal issues such as crossbites. Treatment strategies are suggested across the patient’s developmental stages, from early intervention with orthopedic devices like reverse facemasks, bone-anchored maxillary protraction (BAMP) for growing patients, to surgical solutions such as LeFort osteotomies and distraction osteogenesis for severe hypoplasia in adults. Emphasis on the importance of speech assessment and the potential impact of maxillary advancement on velopharyngeal function, suggesting anterior maxillary distraction for cases where speech is a concern. Ultimately, the chapter outlines treatment algorithms and a decision-making framework based on patient age, severity of the hypoplasia, and existing speech.
