Upper blepharoplasty has traditionally been performed using subtractive techniques that focus on removing excess skin, muscle, and fat. However, this approach may accelerate the natural volume depletion and bone resorption associated with aging, leading to deeper superior sulci, A-shaped deformities, and an overall hollow appearance. This chapter introduces the volumetric contouring concept for upper blepharoplasty, which emphasizes fat preservation, lacrimal gland repositioning, and volume augmentation procedures. This technique seeks to restore the periorbital region’s youthful contour, addressing all involutional changes while preventing hollow eyes and achieving long-lasting aesthetic outcomes.
