Surgery of Minor Salivary Glands

The diseases affecting minor salivary glands encompass a wide spectrum of pathologies, each with its distinct etiology, clinical presentation, diagnostic criteria, and management strategies. Minor salivary gland pathologies encompass a spectrum of conditions including benign and malignant tumors, infections, autoimmune disorders, and inflammatory conditions. Surgical procedures will involve biopsies, excision of benign and malignant tumors, and cystectomies. Minor salivary gland biopsy, usually done on the lower lip where the glandular units lie just below the surface of the labial mucosa, is a low-risk procedure that can provide vital diagnostic information about diseases of salivary parenchyma, such as degenerative and inflammatory diseases, amyloidosis and sarcoidosis. Others will be for diagnosis of neoplasms. Mucocele, a benign mucus-containing pseudo-cyst of the minor salivary gland, is the most common lesion that arises from a minor salivary gland. The most frequent site of occurrence is the lower lip, but lesions also occur below the tongue and in the buccal mucosa. The treatment would be surgical excision of the mucocele along with the associated minor salivary gland. About 40–80% of minor salivary gland tumors are located in the palate, and the most common symptom is a non-tender swelling inside the mouth. Benign salivary gland tumors are typically encapsulated or pseudo-encapsulated, slowly growing, and the overlying mucosa has a normal appearance unless traumatized. The surgery for benign tumors will depend on their anatomic location. Malignant tumors may appear similar to benign tumors early but will become ulcerated, painful, and fixed to underlying tissues. It has been reported that between 60% and 80% of all minor salivary gland tumors on the palate are malignant. Surgical treatment must completely remove all involved tissues with en-bloc resection of the mucosa and underlying bone with at least a 1 cm margin. Dental and oral surgeons must focus on precise diagnosis and individualized care to optimize patient outcomes.
