摘要

Local anaesthesia of peripheral structures and nerves with lidocaine or related drugs can reduce central tinnitus-related activity. The physiology underlaying this phenomenon is the fact that the somatosensory systems and the trigeminal nerves can have a significant impact to the central auditory systems. Inhibition of pathologic afferencies from these structures to the brain is the intention of the described therapeutic procedures. Trigger points in muscles and tendons in head and neck area and the shoulder girdle are examined by specific active and passive tests in a silent chamber and then treated with needle injection of local anaesthesia. The trigeminal nerveTrigeminal nerve plays another role. Well described by S. Shore, this nerve system has enormous influence on the dorsal cochlear nucleusDorsal cochlear nucleus. The deafferentation here can be accomplished by administering the local anesthetic to the otic ganglion. The proper application is done intraorally, which is in contrast to mostly described procedures in the literature. The application of Botulinum toxin is helpful for the palatal myoclonus, when muscles are contracting irregularly. The toxin acts here by blocking the neuromuscular action. Another effect is an antinoceptive one which might also be helpful for inhibitation of central trigeminovascular neurons. The application is done around the ear and to cervical muscles.
