Discharge and Post-hospital Care

A well-planned discharge can reduce the risk of adverse outcomes and readmissions. A multidisciplinary approach and good communication both with the patient and family/informal caregivers as well as primary care throughout the process is essential for a safe discharge. Facilitating patient participation when planning discharge and post-hospital care can increase their health and well-being. This chapter aims to give an overview of best practice in discharge planning, discharge and post-hospital care following fragility fracture and describe different case management models.
