HIV-associated neurocognitive disorder (HAND) is a major neurological complication of HIV. The diagnosis of HAND is done through clinical assessment and exclusion of other potential causes of neurocognitive impairments. No specific biomarkers have been identified for HAND’s diagnosis and treatment response. Because cerebrospinal fluid (CSF) is in proximity to the central nervous system (CNS), it is routinely used to investigate diseases or disorders of the CNS. CSF analysis to determine the biomarkers of HAND is thus of interest, and numerous studies have used the CSF platform to evaluate the neuropathological changes that are present in HAND. CSF biomarkers have also been used as proxy biological markers of HAND severity and prognosis. In this chapter, the CSF as a platform for biomarker discovery of HAND has been explored. The chapter therefore discusses various potential biomarkers at different levels of neuropathogenic pathways that are of relevance to HAND.
