After its first publication in 1997, minimally invasive video-assisted thyroidectomy (MIVAT) has been developed and standardized and received wide acceptance in the surgical community. Several comparative studies demonstrated the safety of the technique, which provides a complete surgical resection with similar complication rates to its conventional counterpart. Besides the improved aesthetic result, MIVAT showed several advantages in terms of postoperative pain and postoperative outcome. Indeed, the magnification of the endoscope enhances the identification of the critical neck structures allowing minimization of the incision length and extent of dissection. An accurate patient selection plays a key role in ensuring the success of MIVAT. In this chapter we focus on the surgical technique, the indications and the main outcomes of MIVAT.
