Exudative age-related macular degeneration (nvAMD) is a leading cause of severe central vision loss worldwide. This chapter provides a comprehensive overview of its pathophysiology, clinical features, and current diagnostic and treatment strategies. The hallmark of nvAMD is choroidal neovascularization driven by VEGF overexpression, often triggered by oxidative stress, aging, and genetic predispositions. Imaging modalities including fluorescein angiography, indocyanine green angiography, and OCT-A play a pivotal role in diagnosis and monitoring. Anti-VEGF therapy remains the gold standard, with emerging long-acting agents and AI-based tools transforming patient management. Prognostic factors based on OCT biomarkers and genotypic variants are discussed, along with complications such as RPE tears, subretinal hemorrhage, and disciform scarring. Despite advancements, long-term visual outcomes remain variable, emphasizing the importance of individualized care and future therapeutic innovations.
