Posterior Lensectomy

Posterior lensectomy consists of extraction of the lens via pars plana, with the use of both the vitreophage in less dense crystallines and the phacofragmentation tip in denser crystalline lens. It is usually used in cases with a very high risk of dislocation or subluxation of the lens, zonular fragility, traumatic cataract with posterior capsule rupture and during pars plana vitrectomy. It allows the surgical procedure to be performed in a closed ocular system, with minimal trauma to the cornea and control of vitreous prolapse. Although rare, postoperative complications are described, such as retinal detachment, cystoid macular edema, ciliary body detachment, glaucoma, epiretinal membrane, and endophthalmitis. With the emergence of the 23G and 25G system in vitreoretinal surgery, posterior lensectomy via pars plana is stabilized as a minimally invasive, safe, and very resolutive procedure as an alternative to phacoemulsification in cases of dislocated or subluxated lens.
