Transoral Robotic Surgery for Obstructive Sleep Apnea Syndrome

Although most OSAHS patients can benefit from the gold standard CPAP therapy, many of them cannot tolerate it, leading to poor compliance. This group of OSAHS patients may benefit from surgery as an effective alternative treatment. The application of transoral robotic surgery (TORS) in the surgical treatment of OSAHS in recent years has shown promising results. With advanced technology, Not all OSAHS patients are suitable candidates for TORS. To reduce the rate of surgical failure and postoperative complications, proper selection of surgical candidates should be made based on clinical history, physical examination, DISE, and radiological imaging. Despite favorable results, robotic surgery, in particular TORS, is encountering some obstacles preventing large scale application. High cost, lack of custom-made instruments for head and neck surgery, and anatomical restrictions that limit the potential for robotic surgery seem to be the principal factors. On the other hand, growing interest in robotic surgery has led to new potential solutions to the obstacles. The development of the single-port technology and application of robotic technology for multilevel upper airway surgery are showing promising results. Further effort will be needed to define the role for this innovative surgical approach, which has great potential in providing positive benefits for OSAHS patients.
