Dominant, Tumor-Related Mesial Temporal Lobe Epilepsy

We present a patient with pharmacoresistant dominant, tumor-related mesial temporal lobe epilepsy undergoing MRI-guided laser interstitial thermoablation therapy. We discuss thermoablation therapy in patients with epilepsy-related tumors. We shed light on the technical aspects of this intervention and its associated neuroimaging findings. We provide a short-list of differential diagnoses for select lesions seen on neuroimaging findings, such as low-grade gliomas, dysembryoplastic neuroepithelial tumors, and cortical malformations. Lastly, we explore semiologies such as autoscopic phenomena and out-of-body experience.
