Non-dominant, Lesional Hemispheric Epilepsy

We present a patient with pharmacoresistant epilepsy secondary to a ruptured vascular malformation who underwent functional hemispherectomy. We discuss the indications and differences between hemispherectomy and hemispherotomy, their clinical outcomes, and complications. We briefly address the pathophysiology of catamenial exacerbation of seizures. We also discuss semiologies such as unilateral tonic and clonic seizures arising from the primary motor and supplementary motor areas.
