Multifocal Epilepsy Due to New-Onset Refractory Status Epilepticus

摘要

This chapter describes a patient with new onset refractory status epilepticus (NORSE) who developed pharmacoresistant multifocal epilepsy and subsequently underwent topectomy and implantation of responsive neurostimulation device. We highlight the different modalities of language mapping, including direct cortical stimulation and single pulse electrical stimulation. We summarize current data regarding cortico-cortical evoked potentials in evaluating functional connectivity within the nodes of the epileptic network. We review the surgical interventions and outcomes in patients with prolonged refractory and super refractory status epilepticus, including NORSE. We examine the proposed mechanisms of acquired epileptogenesis. Finally, we provide a brief overview of the neuroimaging characteristics of patients with NORSE.
