Non-dominant, Lesional Temporal Lobe Epilepsy

This chapter describes a patient who developed pharmacoresistant extra-hippocampal, temporal lobe epilepsy due to cavernoma. This case illustrates treatment options for cavernoma-associated pharmacoresistant epilepsy. We present data on the post-operative outcomes and predictive factors. Additionally, we discuss the utility of intra-operative electrocorticography in resection of the cavernomas. Moreover, we discuss the evidence regarding the optimal timing for surgical intervention in patients with cavernoma-related epilepsy, regardless of their refractoriness. We concisely review the EEG findings in mesial and neocortical temporal epilepsies. Finally, we analyze semiologies such as depersonalization and derealization.
